Clarify Health is designed to support existing care.

Clarify Health is an independent, cash-pay consulting practice led by a board-certified critical care pharmacist. It does not manage conditions, direct treatment, or take over any part of a patient's care. It creates dedicated time for detailed medication and supplement analysis, and returns the result to you.

The Premise

A visit has more to accomplish than a full regimen review.

Patients arrive with prescriptions from several sources, over-the-counter products they may not think to mention, and supplements chosen from outside medicine entirely. Reconciling all of it, assessing interactions across categories, reading laboratory trends against the therapies that affect them, and separating what matters from what does not is a time-consuming exercise — and time is the scarcest resource in a scheduled visit.

Clarify Health takes that exercise on as a discrete piece of work, at the patient's request and expense, and produces something you can act on in a few minutes: a concise summary of findings and prioritized, evidence-based recommendations, with the reasoning available if you want it.

Scope of Work

What a review covers.

Depending on the patient and the questions raised, a review may include any of the following.

  • Medication reconciliation across all sources, including OTC products and supplements
  • Drug–drug, drug–supplement, and supplement–supplement interaction assessment
  • Adverse effect assessment for symptoms of uncertain origin
  • Evaluation of therapeutic duplication and regimen complexity
  • Administration, timing, and absorption considerations
  • Adherence barrier review
  • Relevant laboratory interpretation and monitoring considerations
  • Evidence review for therapies where the literature is genuinely in question
  • Supplement evidence quality and cumulative ingredient exposure
  • Cost and value analysis where data are sufficient
  • Patient education, so your instructions are understood and followed
  • A concise written communication summarizing findings and prioritized recommendations

Clinician Communication

What you would receive.

  • A brief statement of what the patient asked to have reviewed
  • A reconciled list of medications, OTC products, and supplements
  • Findings judged clinically relevant, with supporting reasoning
  • Specific recommendations, prioritized, with an indication of confidence
  • Anything requiring a prescriber's decision, stated as a question rather than an instruction
  • Monitoring suggestions where relevant

Communication occurs only when the patient authorizes it and when it would be useful. Recommendations are exactly that — whether to accept, modify, or decline any of them is your clinical judgment.

Boundaries

What Clarify Health does not do.

  • Diagnose disease or manage conditions
  • Replace primary or specialty care
  • Direct patients to discontinue prescribed medications
  • Provide urgent or emergency care
  • Order or alter treatment without the responsible clinician
  • Sell, dispense, or recommend any product it profits from

Patients are told plainly that their physician remains their physician, and that any change to a prescription is a decision for them and their prescribing clinician.

Who Performs the Work

Doctor of Pharmacy and Board Certified Critical Care Pharmacist, practicing clinical pharmacy since 2009 in trauma and critical care at a major academic medical center. Every conclusion and recommendation in a Clarify Health report is reviewed and approved by the pharmacist.

Referring a patient or asking a question

Clinicians are welcome to get in touch with general questions about scope, process, or what a report contains. Patients enroll themselves through the secure client platform — there is no referral requirement and no referral fee in either direction.
